2019 OFOR Fall Retreat Registration Form
Greetings! We're excited you'll be participating in the OFOR fall retreat! We appreciate you filling out this form so we can work with you and other participants to prepare to tend to everyone's needs. Be sure to complete a registration form for everyone in a family group.

All sleeping rooms have sheets, blankets, and pillows. You may wish to bring an additional blanket or pillow for your comfort. We’re suggested that participants bring individual towels for bathing, which will help us minimize cleaning and laundry for the event.

Some of the rooms have double beds. Is there someone that you would feel comfortable sharing a bed with?
We will be preparing group meals at the Retreat. We will have participants bring foods and beverages to share for meals.
Please indicate below what you will be able to bring or a meal you willing to prepare
Friday Dinner: Vegetables for soup, breads
Sat/Sun Breakfast: granola, yogurt, muesli, toast, eggs, fruit
Sat lunch: Vegetables for salad, sandwich makings - hummus, pb and j, cheese, sandwich meats etc.
Sat. Dinner: Taco bar - tortillas, lettuce, tomatoes, onions, beans, cilantro, olives, cheese
tea, coffee, apple cider
Food allergy (if so, please indicate what you are allergic to below under other)
Registration fees will be collected at the Retreat. We are asking all adults (18 years and over) who wish to attend for the entire weekend to pay sliding scale of $25 to $75 as able. If paying $25 will present a hardship, please let us know that.
