2025/26 Ask a Mathematician Request Form
Please note: this form is used to request a visit, it is NOT confirmation of a booking. If your class is selected for a visit you will receive an email from a member of our team to provide further details.

We are very excited to bring our Ask a Mathematician program back (virtually) into your classrooms for the 2025/26 school year.

This form will collect some information on your classroom so we can attempt to match you with one of our Ask a Mathematician experts.

If you have any questions or concerns please email Pamela Brittain, K -  12 Academic Coordinator at pbrittai@fieldsinstitute.ca

Math Level (approx) of Students (if known)
Given the past few years we understand that many students may not be at grade level with their math skills. In order to help our speakers properly prepare their talks we are asking for your insights into your class' level of math knowledge. If you know this information please let us know, in general, if your students are at, below, or above grade level in their math skills or if there are any special considerations we should take into account.
Number of Students *
Technical Capabilities / Requirements *
As this is a virtual program we need to know a little about the technical capabilities of your classroom so we can plan accordingly. Please check all that apply to your classroom.

Have you participated in the program before? *
Optional Diversity Questions
The AAM program relies on funding and support from granting agencies and one of the things they are most interested in is the diversity reach of the program, particularly to groups that are typically underrepresented in mathematics. The following three questions are optional and any information provided will not be used in determining whether or not a visit to your school is scheduled. The information is collected for statistical purposes only.
What percentage of your class (approx) is female identifying?
What percentage of your class (approx) would consider themselves as one of the following: Black, Indigenous, or Latin American?
Anything else about the diversity profile of your class you would like to share?
A copy of your responses will be emailed to the address you provided.
