Complete the following form to place your order for a T-shirt or sweatshirt. T-Shirts cost $20 each and sweatshirts cost $30 each.
The order will not be finalized until payment is received. Payment must be submitted by October 6th. If paying with a check it can be made out to Mt. Carmel COGOP. Payment can be given to Pastor Jordan or Kaitlyn Faircloth.
Items are expected to arrive between October 20th and 27th.
