Marching Band Excused Absence Request Form (2022)
NOTE: THIS FORM SHOULD BE USED ONLY TO COMMUNICATE EXCUSED ABSENCE REQUESTS RELATED TO MARCHING BAND REHEARSALS AND PERFORMANCES OUTSIDE OF THE NORMAL CLASS SCHEDULE. PARENTS/GUARDIANS SHOULD CONTACT THE STUDENT'S ENSEMBLE DIRECTOR REGARDING CONCERT BAND ABSENCES. ABSENCES THAT OCCUR DURING THE SCHOOL DAY SHOULD BE REPORTED TO THE CAMPUS ATTENDANCE OFFICE.

All requests for an excused absence must follow the 3-step process described below. Requests will only be considered if all steps are completed. This process should be completed at least 7 days prior to the absence date whenever possible. If the student becomes suddenly ill, please complete this process within 24 hours of the missed event. The Allen High School Band & Colorguard will apply all Allen ISD policies regarding excused absences. Please see below for approved excused absences (found on pages 34-37 in the Allen ISD Student Handbook for details).

(1) A PARENT/GUARDIAN must send an e-mail to their student's ensemble director stating the date of absence, duration of absence, and the reason for the absence. Attach a doctor's note if necessary.
(2) The STUDENT must communicate with their section leader to let them know they will be absent for a particular rehearsal.
(3) The student or parent must complete this form. If you will be out for a range of dates, you must submit a form for every date you will miss.


NOTE - Excused absences as determined by the AISD student handbook include:
          DOCUMENTED HEALTH-CARE APPOINTMENTS: A note from the health-care provider must be submitted upon the student’s arrival or return to campus.
          RELIGIOUS HOLY DAYS: parent note required
          REQUIRED COURT APPEARANCES: court note required
          ACTIVITIES RELATED TO OBTAINING U.S. CITIZENSHIP: legal document required
          COLLEGE VISITS: An 11th or 12th grade student’s absence of up to two days related to visiting a college or university will be considered an exemption, provided this has been authorized by the board under policy FEA (LOCAL), the student receives approval from the campus principal, follows the campus procedures to verify such a visit, and makes up any work missed.
