Omni Membership Change Form
This form is designed to help you make changes to your Omni membership status.

Please read carefully before submitting:

  1. Deadline: Please submit all membership change requests 48 hours before the end of the month (E.g. If you'd like to request a change for October, submit your request by September 28th) – This is because we process all requests on the second to last day of the month to ensure changes take effect on the 1st of the following month.
  2. Processing Time: Membership changes will be processed on the second to last day of the month.
  3. Limited Requests: Please keep in mind that Omni is generally short-staffed, and each membership change needs to be manually entered. We kindly ask that you limit your membership change requests. 
If you experience any issues (such as being charged despite your membership being on hold), please email info@theomnimovement.org.
