This form is designed to help you make changes to your Omni membership status.
Please read carefully before submitting:
- Deadline: Please submit all membership change requests 48 hours before the end of the month (E.g. If you'd like to request a change for October, submit your request by September 28th) – This is because we process all requests on the second to last day of the month to ensure changes take effect on the 1st of the following month.
- Processing Time: Membership changes will be processed on the second to last day of the month.
- Limited Requests: Please keep in mind that Omni is generally short-staffed, and each membership change needs to be manually entered. We kindly ask that you limit your membership change requests.
