Sharrett Counseling Growth Group Registration Form
Thank you for your interest in the Sharrett Counseling Growth Groups. This is a professional counseling service that I believe you will find to be a great benefit for your personal growth and/or for the growth of your child's social and emotional leadership skills. 

You can register any time during the month. New groups will start each month for newly registered participants.

Please complete this short form to register your child/adolescent or yourself for this program, and I will be in touch with you about a consultation session so that we can discuss how to best serve you and/or your child.

Students will be grouped in the best ways for their individual success and group success. Two sessions will happen per month at a time that works for all members of the group. Please check all the boxes for the preferred times your child can meet. I will be in touch about the scheduled meeting times. *

If you do not have a Paypal account, I can send a direct request for payment to your email address, and this should allow you to pay via Paypal.

Total cost of the program is either 300 euros or 400 euros, depending on group or individual registration.
 
Please pay 100 euros when you register as a deposit. The remaining amount can be paid after the second month and the four month, unless you want to do it differently.

If additional services or counseling sessions are needed, appropriate fees can be discussed and applied.  

To complete the registration process, please follow these steps:

1) Submit this registration form.

2) Complete the informed consent (separate form) when it is sent to you.

3) Schedule a brief consultation (20-30 minutes online) with Lawrence to discuss your interest and hopes for this program. This will help determine if this program is a good fit for you and/or your child.

4) Pay the 100 euro deposit via PayPal to sharrettcounseling@gmail.com
