I give my son permission to attend the Cathedral Prep Weekends that will run from October, 2023-June 2024 held at St. Joseph’s Seminary, 201 Seminary Avenue, Yonkers, New York 10704. The weekends will run from Friday evening through Sunday afternoon. I understand that my son will make use of the gymnasium and engage in athletic activities if he so wishes. I am also aware that on occasion, my son will be transported to events/shrines/attractions, etc. via seminary vehicle. In order to ensure that each student enter the fully of the program we ask that students not use: Cell phones, or any media players. For everyone's safety we ask that students not bring knifes, or any other weapons, or any drugs or alcohol.  If students are found with these items the items will be confiscated and you will be contacted in order for you to pick-up your son. *
I give the Archdiocese of New York and any Catholic entity within the Archdiocese of New York, permission to use my child's image and likeness in any creative work for the archdiocese or any entity within it.  Furthermore, I give them permission to use any audio/visual interviews, and any pictures that were taken of my child. I understand that I will not receive financial compensation, goods or services in exchange for providing the Archdiocese of New York with any of the aforementioned. *
