In the past there has been some confusion that resulted in currently adopted textbooks being sent to the Warehouse and recycled, resulting in expensive textbook bills for schools.
1) This form must be filled out for any out of adoption textbook titles BEFORE you send any textbooks to the warehouse
2) You'll receive a label to print and put on EACH box that will be sent to the Warehouse with out of adoption textbooks
3) The Warehouse will not pick up out of adoption textbook materials without this sheet attached to each box

*Please remember that consumables do NOT need to be added to this form. Label box as "Unused consumables" and send to warehouse with other items needing to be picked up.*
