T-Shirt Included. The camp fee of $30 may be mailed to “Lady Cat Softball Camp” Attn: Railee Shirley, 200 W Manchaca Ave. De Leon, TX 76444 or it may be turned in on the first day of camp.  If a check is written please make it payable to Railee Shirley.

I hereby register my child for the above camp and authorize staff to direct her in participation in camp activities. My child has no medical or emotional conditions which may affect her ability to safely participate in the program. The staff is authorized to attend to any health problems or injury my child may incur while attending the camp. I further acknowledge that De Leon Lady Cat Softball Camp; coaches and anyone associated with the camp will not be held liable for any damages from injury  or illness sustained at the camp.
