Farm Plaque Project
All donating farms will be listed on the Dairy Shrine webpage with a link to their farm information. All proceeds from the plaque will go toward upgrades and ongoing maintenance of the museum. Upon moving forward with the donation, a high resolution logo and website URL will need to be sent to info@dairyshrine.org. Please send payment to Dairy Shrine, PO Box 68, Fort Atkinson, WI 53538. Which option would you like to choose? *
  • Farm Listing:  A listing of farms will be on display in the museum as contributors to the project. This will be a semi-permanent listing that will list the name of the farm and location. Donation: $100
  • Farm Plaque:  A 6" x 9" wooden plaque will be put on display inside the museum. The farm logo or name will be etched into the plaque. In addition, a QR code will be etched into the plaque that, when scanned, will take the person directly to the farm's website or social media page. If the farm doesn't have a web page or social media page, the QR code will take the user to a Dairy Shrine website where a farm-supplied paragraph description on the farm will be available. Donation: $500 to be renewed every five years.
  • Lifetime Farm Plaque:  The same plaque will be on display indefinitely. Donation:  $1,000
