Student Schedule Change Request Form
If a student is need of a schedule change for any reason, this form MUST be fully completed. All schedule change requests must be approved by the Principal and/or his designee prior to any changes being made. You MAY be contacted with any further questions. If you do not receive a response within five (5) days then your request has been denied. Please submit only ONE request.

The deadline for any schedule changes in one week into the Spring 2020 semester, which is Friday, January 10, 2020.

Your schedule change request will be considered if it meets the following criteria:
1. Your request meets one of the provided reasons listed below in the form.
2. A signed schedule request form was completed and returned to your counselor during the 2018-2019 school year
