INSTRUCTIONS — PLEASE READ CAREFULLY
IMPORTANT: This form is for students taking multiple math courses with conflicting common exam times. Students will be issued an alternate exam time and location, NOT a new exam date. This application must be submitted no later than the Wednesday prior to the exam date. The deadlines for the Spring 2017 common exams are: 4 credit courses (Math 108, 110, 111, 111, 112, 112H, 213, 222, & 222H) Exam Date Application Due Date 02/15/17 02/08/17 03/08/17 03/01/17 04/19/17 04/12/17 3 credit courses (Math 107, 211, 333, 337, & 337H) Exam Date Application Due Date 03/01/17 02/22/17 04/05/17 03/29/17 Please note that you only need to apply one time per semester for each exam conflict. Once you have received your validated conflict form, you may use it for the remainder of the semester. Please allow two business days for processing. Please DO NOT contact the Math Department to inquire about the status of your application until two business days have passed. In order to process your application, you must complete the following steps: 1) Provide the course name and section number of each course which conflict with you common exam times. 2) Complete and submit this application. 3) You will be notified by email to come to the Math Department – Cullimore Hall, Room 606 – to pick up the validated exam conflict form. You must bring your NJIT student ID card; your form cannot be validated without it. 4) Bring a copy of the validated exam conflict form with you to the exam. You will not be able to take the exam without it.
