2) Pay Registration Fee via PayPal, Venmo or by mailing a check to Stephen Knox. (An option for paying via Square will be available later in June)
3) Ensure that hotel and flight reservations are made by members (independent from this registration form).

Your time in Kentucky will be spent among the beauty of the Bluegrass country! Activities listed below are for ALL attendees (group events).

These activities are for the spouses/significant others, while SCN members and guest presenters are in the Scientific Session.

Registration Fee: $850 for SCN Members; $800 for Spouse/Significant Other; there is no charge for Guest Presenters. Select the option appropriate to you (and a guest(s), if applies.)

2) Member AND Spouse/Significant Other (two people) - $1650

3) Guest Presenter only (one person) - $0

4) Guest Presenter AND Spouse/Significant Other (two people) - $800
