By completing this form you are committing to purchase a senior page or partial senior page. You will need to provide pictures and any messages you would like to include and a deposit prior to December 6, 2019. A minimum of 50% deposit required by Dec. 6, 2019. Payment is due in full before March 6, 2020.
If you need to cancel or make changes to your order send an email to nicole.ford@polk-fl.net, you must do so before December 6, 2019. All sales are final at that time. Yearbooks will arrive in May 2020, before seniors graduate!
