Application Eligibility and Instructions
The scholarship deadline is Thursday February 29 , 2024 at 5pm.
Scholarships range from $1,000 to $2,500
Non-electronic submissions will NOT be accepted. Modifications to this form or submission in other formats will NOT be accepted.
All fields must be completed as required. Blank or incomplete information may result in disqualification from the process .
Official transcripts must be postmarked by the scholarship deadline Thursday, February 29, 2024, at 5pm and mailed to University Health Professionals, Local 3837, 270 Farmington Ave. Suite 109, Farmington, CT. 06032 Attn: Scholarship Committee. One letter of recommendation only must be received via e-mail by the scholarship deadline, Thursday, February 29, 2024.
Resumes should be sent via e-mail and must be received by Thursday, February 29, 2024, at 5pm. Please do not send extraneous information other than what is required for the scholarship application.
The correct essay question must be answered for qualification. Please read carefully and select the appropriate essay question (Member, High-school dependent, College dependent).
Applicants are not eligible for the same UHP Dependent Scholarship in consecutive years.
Applicants are only eligible for the UHP Member Scholarships if they have never won that specific award. Members are eligible to apply for a different member scholarship even if they have previously won a member scholarship.
All Scholarship applicants must be a UHP member or dependent of a UHP member in good standing at the time the scholarship application is received to be accepted for consideration by the scholarship committee.
*Member in good standing (A signed membership card, dues paying member).*
Due to the large volume of applicants, we are unable to confirm receipt of any materials.
Selected Scholarship recipients will be notified via email no later than Thursday, April 4, 2024. Due to the large volume of applicants, we are unable to notify all applicants of their status.
Scholarship Awards will be awarded at the UHP Annual Business Convention, Friday, April 12, 2024. It is not required for scholarship recipients to attend, however their attendance is preferred. The physical award will be mailed to the scholarship winners home address unless a mutual time has been agreed upon for an in person pick up. In person pick up will be at the discretion of the scholarship chair.
