This is the digital version of the printable approved Referee Evaluation form on the CVOA Documents website page.

Please use this form to provide positive or negative feedback about your partner following a match. It can be used for single or multiple matches by officials, Area Director, and mentors observing a match. If you would like to request an evaluation by someone, it is best to ask them prior to the match so that they can be observant during the match.

Your partner will not receive a copy. It is recommended that you discuss your observations with them during your post-match debrief, if possible.

Please note that these responses may be used for post-season selection and for determining ratings. Responses will only be visible to your assigner, Area Director(s), and the CVOA Executive Committee.

Important: After an especially difficult or contentious match, please call or text your assigner immediately to provide a heads-up to preempt a possible call from the coach or AD. Do not depend on this form when time is of the essence.
