‘I understand that if I am accepted into the Fraternal Order of Police, Overland Park Lodge #21, I agree to abide by the Constitution and By-Laws of the Lodge. If my membership should be revoked or discontinued for any cause, other than retirement while in good standing, I do hereby agree to return to said Lodge my membership card and any other material belonging to the Lodge, or any item(s) bearing the Fraternal Order of Police name, logo, or insignia. I further understand before I am eligible for the Legal Defense Fund I must be in receipt of my FOP Membership Card and that by submitting this form, I consent to receive electronic communications from the Lodge regarding official business.'
