Request V.A. Education Benefits Certification
In order to be certified for VA Education Benefits at North Shore Community College, you must complete this form.  It is the responsibility of the student to complete this form every semester after registering for courses.

This will provide us with accurate information, and allow us to be more proficient in our certifying.

We cannot certify your enrollment until you register for courses and all documentation has been submitted to Veterans Services (Chapter 33 COE, NG Tuition & Fee Waiver, Chapter 35 COE, CH31 PO was received, etc.).

If you do not have remaining VA benefits, not using your VA benefit, using free college, or using another form of payment for your tuition & fees, please continue to complete this form.

If you have any questions, please reach out to Veterans Services.

How many credits are you registered for? *
By checking the box, I certify that I have read the policy on course applicability and all the courses I am registered for will satisfy requirements outlined by my degree evaluation. *

The VA will only pay for classes that are REQUIRED for your degree.

Only courses that satisfy requirements outlined by the curriculum guide or graduation evaluation form can be certified for VA purposes.

If a student takes a course that does not fulfill a program requirement, it cannot be certified for VA purposes. Excessive free electives, for example, cannot be certified.

FAQ:  https://www.northshore.edu/veterans/resources/faq.html

Required
By checking the boxes, I certify that I have read the policy on Repeating Courses *
Classes that are successfully completed may not be certified again for VA purposes if they are repeated. However, if a student fails a class, or if a program requires a higher grade than the one achieved in a particular class for successful completion, that class may be repeated and certified to VA again.

Required
Schedule changes, stopped attending, never attended or withdrawal from a course(s) *
It is the responsibility of the student to notify Veterans Services by email if you change your schedule, stop attending or never attend a course(s).

After we certify your enrollment with the VA and you do not notify us of any of the above changes, this will most likely create a VA debt and/or a NSCC debt for you.

The VA does not pay for course(s) or housing allowance if you stop attending, never attended or withdraw from a course(s).  It is the responsibility of the student to notify Veterans Services by email of any changes.

When you receive an email from Academic Affairs stating you never attended or stopped attending a course(s), Veterans Services receives the same notification and your VA enrollment certification will be amended.

When ordering your AF transcript, have it sent to NSCC to be evaluated.  DO NOT send it to the Center for Alternative Studies, they do not evaluate college transcripts for credit.  This will only delay in evaluating AF transcripts.
