Carriage Crest Elementary PTSA Awards 2017-2018
Outstanding Educator, Golden Acorn, and Outstanding Advocate
The CC PTSA is seeking input from the Carriage Crest Elementary staff, parents, students, and
community members to identify exceptional advocates and volunteers within our school community.
You may nominate more than one person for each award. *You do not need to nominate for all 3
awards to participate.

Golden Acorn Award
Recognizing a volunteer, teacher, or Carriage Crest staff member who has gone the extra mile to make a difference for our students at Carriage Crest. It is a person whose hard work goes beyond expectations, not just an individual who has donated time to help in his/her child’s class or a staff member who is doing a good job. This award is given to recognize service “above and beyond” a particular job description.
Golden Acorn Award Nomination:

Outstanding Educator Award
Given to a teacher, assistant, custodian, bus driver or
administrator who goes beyond the normal expectations of the job to make the learning environment
at Carriage Crest fun, nurturing, beneficial, and/or challenging.

Why are you nominating this person for the Outstanding Educator Award?
Outstanding Advocate Award
Honors an individual for his/her advocacy work on behalf of children and youth. Recipients demonstrate a commitment to helping create strong policies relating to the health, welfare, safety, and education through PTA legislation, private organizations, education and/or community outreach programs. Staff members or volunteers who speak up for students or who try to change things for the benefit of children are considered advocates.

A nomination for these awards is a wonderful way to say, “Thank you for all you do!”
All award winners will be honored at school and with an invitation to the annual Founder’s Day Event. We
hope you will help by nominating award recipients. Our PTSA awards committee will make the final
