I, (hereinafter Applicant) acknowledge that I am over the age of 18,
mentally competent, and of sound mind, and I am hereby applying for employment with
A Morris Industries Inc, DBA AM Irrigation, a Florida corporation (hereinafter Company).
As a condition of my employment, I hereby authorize Company, and its designated agents and
representatives, to perform a background screening of my history that the Company, in their
discretion deems necessary and appropriate to determine my fitness to be employed by
Company. I further authorize Company to generate an investigative report for employment
purposes. I understand that the scope of the report will include, but not be limited to, verification
of social security number, driving records, criminal traffic history, worker's compensation claim
history, current and previous residential history, character references, credit reports, and any
other public records.
I further authorize any individual, company, firm, corporation, or public agency to divulge and
release any and all information, verbal or written, pertaining to me, to the Company, or its agents
and representatives. I further authorize the complete release of any records or data pertaining to
me which the individual, company, firm, corporation, or public agency may have, to include
information or data received from other sources provided that any information Company
receives is maintained in a confidential manner in order to protect my personal information,
including, but not limited to my address and social security number.
By signing this document, I am providing the Company with my explicit written consent for an
initial background screening related to my fitness for employment by Company, as well as any
subsequent screenings, or follow up screenings, Company may require prior to and during my
employment.
