Lifespan Spiritual Education Registration for Children and Youth 2018-2019
Hello and welcome to the Unitarian Church of Montpelier!

Thank you for taking the time to register your child or youth. This year our program is transitioning from the title Religious Education (RE) to Lifespan Spiritual Exploration (LSE). We are excited to continue creating connections, space for fond memories to form and opportunities for spiritual growth for all ages. We take the well being and safety of the children in our UCM community very seriously. The information you provide on these forms enables us to better meet the needs of the children and youth, along with your own needs as their caregivers. This form is for any kids or youth participating in our Lifespan Spiritual Exploration programs including:

-nursery

-Sunday morning programs for preschool-6th grade

-Our Whole Lives or OWL

-High School Youth Group

-Youth who are on the Child Care Team (either for nursery or extra child care options).

If you would like to set up a visit to the church with your child or youth before you come for the first time, please email me at ucm.dre (at) gmail.com and I'm happy to arrange a time to give you a tour and talk through any questions you might have.

Photo Permission *
I/we give permission for my child/ren to be photographed and/or videotaped while participating in LSE programming and church related activities. These photos may be used within the church and/or published in church printed materials and/or on the church website or Facebook page. I understand that NO CHILDREN WILL BE IDENTIFIED by name.

I give permission for my child(ren) to go outside the church and/or off the church grounds for short walks in the company of at least two adult LSE team members as part of an LSE activity or class.

Reminders and Requests
If you have more than one child in the LSE program, feel free to fill out the sections below only once.
For parents of children 3rd grade and younger: Parents need to pick up any children in 3rd grade or younger by 10:10 a.m. (for the 9 a.m. service) or 12:10 p.m. (for the 11 a.m. service) or immediately after the service if the service runs late.
If your child is choosing not to participate in Sunday morning LSE programs, parents are responsible for ensuring their safety and appropriate supervision in the building and on the grounds.
Important reminder: Please don't bring your child(ren) or youth to the church if they have a fever, have been vomiting within the previous 24 hours, or have a communicable illness in the contagious stage (ex. Hand Foot Mouth disease). *

Ways You Can Support the Lifespan Spiritual Exploration Program
We are delighted to have you as part of our religious education family! It takes a lot of caring hands and hearts to run our program. Some churches have a cooperative model, where at least one parent/caregiver is helping most Sunday mornings. At UCM we don't have any mandatory level of participation, be we do hope that you’ll join in as you're able when volunteers are needed.
