We are so excited to welcome your little one into our Preschool Program at Kids Garden Summerville! If you could please fill out a little information on your child below, that would be great!

In addition to this form, please make sure you have Registered with us on our website.
We are also required to have a DSS 2900, an All Policies/Medical Authorization and a SC DHEC Immunizations Form (or DHEC Exemption Form). These forms are under the Enrollment Forms Link on the website and are also available at the front desk as well, please allow a few minutes for paperwork at your first drop off. :)
