We are dedicated to helping individuals with special needs acquire the skills they need for successful, independent, productive, and fulfilling lives in their community. As such, our admissions process is designed to ensure a match between the needs of an applicant and the services offered by GoodLife. This "Initial Screening Form" is the first step in that process.

All applications are subject to review by the GoodLife Initiation of Services Committee. If the IOS committee agrees that GoodLife is able to support the individuals' support needs within GoodLife's models of care and the applicant has met the minimum requirements, the individual will be placed on a waitlist until GoodLife determines that an appropriate placement becomes available. Approved individuals will be offered placement when a vacancy occurs upon (1) meeting the eligibility criteria of that specific program and/or home; (2) roommate compatibility; (3) and agreeing to GoodLife's terms of service and financial agreements. Internal movements will take priority for all vacancies.
