As parent or legal guardian of the Student, hereby give permission to officials of the Durham Public Schools to disclose my student’s DPS email address to the New York Times for the purpose of a free online subscription to the New York Times.  

I also consent to the following New York Times user agreements on behalf of the Student:

I acknowledge that this form constitutes my written consent to the release of personally identifiable information protected under the federal Family Educational and Privacy Rights Act (FERPA), state law, and applicable Durham Public Schools policies and my written consent to the two New York Times user agreements referenced above. I understand and certify that: I am the parent or legal guardian of the Student and that I have authority to execute this document regarding the Student’s education records and to consent to the New York Times agreements referenced above on the Student’s behalf; I have the right to revoke this authorization and consent at any time by providing my signed written notice to the Student’s school principal.  Absent such notice, this authorization shall expire within one (1) year of the date indicated below.

THIS INFORMATION RELEASE IS SUBJECT TO THE CONFIDENTIALITY PROVISIONS OF APPROPRIATE STATE AND FEDERAL LAWS AND REGULATIONS WHICH PROHIBIT ANY FURTHER DISCLOSURE OF THIS INFORMATION WITHOUT THE SPECIFIC WRITTEN CONSENT OF THE PERSON TO WHOM IT PERTAINS, OR AS OTHERWISE PERMITTED BY SUCH REGULATIONS.

If you already have a NYTimes.com account associated with such email address, log in with those credentials. If you forgot your password, visit nytimes.com/forgot to reset it.

Check your email inbox for our confirmation message.  Click on the link in your confirmation message to validate your email address and claim your Pass.  If the confirmation email didn’t arrive, check your spam folder. If it isn’t there, send an email to edu@nytimes.com from your school email address to request confirmation.
