SRHS Foundation Funding Requests
If you would like to utilize Foundation funds, please submit this form with the requested item(s), pricing information and need date a week prior to the meeting so that there is time to review and vote upon the request. Meetings are scheduled on the following dates:

3/14/22
4/11/22

Process after approval:
1. Notification of decision will be given to the contact person within 5 days of the Foundation board meeting.
2. Submit receipts (or copies) using the "Foundation Check Reimbursement Form" electronically to the Foundation Treasurer (chris@ark-management.com).
3. Any funds not accessed within 60 days will revert back to the Foundation and a new online request must be submitted.
4. Funds can only be used on originally requested items and reimbursement will not exceed the amount originally approved.
