If you would like to utilize Foundation funds, please submit this form with the requested item(s), pricing information and need date a week prior to the meeting so that there is time to review and vote upon the request. Please follow the protocol of running all requests through your department head first and through Dr. Lawson prior to filling out the request. Meetings are scheduled on the following dates: TBD
Process after approval:
1. Notification of decision will be given to the contact person within 5 days of the Foundation board meeting.
2. Submit receipts (or copies) using the "Foundation Check Reimbursement Form" electronically to the Foundation Treasurer (firstname.lastname@example.org
3. Any funds not accessed within 60 days will revert back to the Foundation and a new online request must be submitted.
4. Funds can only be used on originally requested items and reimbursement will not exceed the amount originally approved.
