Yahrzeit Memorial Tablet Order Form

Nameplates are cast in bronze (2 x 10 inches)

Memorial light next to nameplate will be lit the Shabbat evening of the week preceding the Yahrzeit and will remain lit for one week. Please note it takes 8 weeks to order a plaque.

Inscriptions may be in English or Hebrew or both
