If you haven't filed your Colorado income tax return, but either received a refund check or correspondence from the Colorado Department of Revenue to verify your identity OR if the IRS has requested that you file an identity theft affidavit, please use this form to report that you may have been a victim of identity theft for your Colorado income tax account.
