REPORTING STUDENT ABSENCES -- PLEASE READ ALL OF THE INFORMATION BELOW CAREFULLY PRIOR TO SUBMITTING THIS FORM!

Yearly cumulative attendance is reflected on student transcripts. It is important that we hear from a parent/guardian if a student will be absent an entire day, arriving late to school, or leaving early from school.  Failure to report such attendance related information within 24 hours of the absence date, will result in the student being truant (unexcused and absent).  Specific attendance information can be found in the CHS Student Handbook by clicking the link below. If you are requesting your child be excused within 30 minutes, please contact the main office directly at (734) 433-2201. Thank you.

ONLY PARENTS LISTED AS HAVING CUSTODY OF THE STUDENT IN POWERSCHOOL MAY SUBMIT THIS FORM!

Falsifying records including, but not limited to, a parent signature, staff signature, misrepresentation on the phone, etc., will be treated as forgery.

In all cases, the person being misrepresented will be notified, plus:
  • 1st offense: 1 day ISS or OSS plus loss of privileges such as hall passes
  • 2nd offense: 2 days ISS or OSS plus parent conference
  • 3rd offense: 5 days ISS and/or OSS plus parent conference

If your child is going to be absent for multiple days, we encourage you to call the CHS main office at (734) 433-2201 and select option 1 to report this. Otherwise, please submit this form for each day your child will be absent.

UPDATED TARDY POLICY:

Unexcused Tardy:
  • 1st and 2nd offense: Warning by teacher to student
  • 3rd offense: One 25 minute office after-school detention plus teacher will contact parent or 25-minute detention with teacher starting 30 minutes before the start of first hour plus teacher will contact parent. (Teacher choice, not student’s choice)
  • 4th offense: Referral to office, one-hour detention (plus parent contact from administration)
  • 5th offense: 2 hour Saturday detention (8:00-10:00 AM) (plus parent contact from administration)
  • 6th offense: 4 hour Saturday detention and parent conference
  • 7th offense and beyond: ISS/OSS with a possibility of additional consequence
1st Hour Excused Tardy: (NEW FOR 2025-26)
  • 5th Excused Tardy: 1 hour after-school detention
  • 7th Excused Tardy: 2 hour Saturday detention
  • 10th Excused Tardy: 4 hour Saturday detention and parent conference
Medical exceptions can be made with written documentation submitted to CHS administration. It is encouraged that parents send in medical notes for excused tardies for first hour so the office can document these in PowerSchool.

FOR STUDENT-ATHLETES:
As a reminder, students MUST be in attendance ALL DAY on athletic game days and at least  the SECOND HALF OF THE DAY on practice days in order to participate that day. Students are allowed to participate for absences reported ahead of time due to reasons such as appointments, bereavement, and college visits as determined by the athletic office.  A "family/personal" absence does not permit students to participate in an athletics practice or game unless approved by the athletic office.  DOCUMENTATION IS NOW REQUIRED FOR ALL ABSENCES ON A GAME DAY AND SHOULD BE SENT TO THE ATHLETIC OFFICE IN ADVANCE.
