PM D-Clearance Request Form
D-Clearance stands for departmental clearance.  Classes with a "D" in the section number i.e. 41073D, have D-clearance restrictions and students must request and receive clearance before registration. Section numbers with "R" are not restricted and are open for graduate students to directly register in.

This form only allows for up to three (3) D-Clearance requests for courses with the PM prefix per form.

Do not use this form for any MPH Online course, noted with a yellow triangle. Students may contact Chrystal Martinez to petition to enroll in MPH Online classes under section 407 at ccromero@usc.edu.

For MPH courses such as PM 502 and PM 508, please email Linda Mendoza Lopez for D-clearance at lindam@usc.edu.
