COVID-19 Visitor Health Screening Form
The safety of our families, staff and visitors is a top priority. We are closely monitoring the evolving COVID-19 pandemic and will periodically update our guidelines based on current recommendations from the Center for Disease Control, the World Health Organization and State and County officials. To prevent the spread of COVID-19 and reduce the potential risk of exposure, visitors must complete this questionnaire honestly and accurately before entering one of our school buildings.
1. Are you currently suffering from any of the following symptoms – fever (100.0 or higher) or chills, cough or shortness of breath/difficulty breathing, nasal congestion, runny nose or sneezing without other known cause, headache, fatigue, muscle or body aches, sore throat, new loss of smell or taste and/or gastrointestinal problems including stomach ache, nausea, vomiting or diarrhea? If yes, access is denied. You may come to campus when (a) it has been 10 days after the first symptom AND (b) you have been symptom-free without medication for 48 hours.
2. In the last 14 days, have you had close contact with someone diagnosed with COVID-19? If yes, access is denied. You may come to campus when (a) it has been 14 days after the contact (b) it has been 10 days after your first symptom (if you suffered any symptoms listed above) AND (c) you have been symptom-free without any treatment/medication for 48 hours. If you are fully vaccinated, you are not required to quarantine and may come to campus.

3. Have you returned from international travel within the last 14 days? As of January 2021, all individuals who travel internationally must quarantine for 7 days. After the 7-day quarantine period, you must get a PCR test with a negative result. If you have travelled internationally and have not quarantined or received a negative PCR test, access is denied. You may return to campus when you received a negative PCR test dated at least 7-days after your return from international travel.

Temperature. If your temperature is 100.0 or above, access is denied.
If you have passed all the above screening questions, you may come to campus. When you enter the building, please wear a face mask, use hand sanitizer and proceed to the front desk to check-in.
