In lieu of paper audition forms, we are going digital! Once a parent has filled out this pre-registration information, actors will only need to submit their top 3 choices of character in person to save time!

This show is for actors aged 8-18 only!

Dates: We will meet Monday evenings from 6-8 PM starting on Monday, August 24 until tech/show week, during which attendance will be mandatory Monday-Sunday (December 7-13).

Show dates will be:

Friday, December 11: 7 PM (Call time is 6)

Saturday, December 12: 7PM (Call time is 6)

Sunday, December 13: 3PM (Call time is 2)

Consent for Image Release: By entering my name below, I am electronically signing and agreeing to the release of my child's image if any photos are taken at theatre class. These photos may be shared with me and on social media for the theatre. RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT

In consideration of being permitted to participate in a production of Big Cheese Theatre, I, for myself, my personal representatives, heirs, and assigns, do hereby release, waive, discharge, and covenant not to sue Big Cheese Theatre, its owners, officers, employees, and agents from any and all liability, claims, or demands for personal injury, sickness, or death, as well as property damage and expenses, of any nature, occurring during my participation.

ASSUMPTION OF RISK

I expressly and specifically assume the risk of injury or harm in the activities and release Big Cheese Theatre from all liability for injury, illness, death, or property damage resulting from the activities.

INDEMNIFICATION

I also agree to hold harmless and indemnify Big Cheese Theatre from any and all claims, suits, or damages, including attorney's fees, brought by anyone else arising from or related to my participation.

Parents are required to enter the premises to drop off and pick up their child each rehearsal. (Unless they are old enough to drive themselves!)
