Please use one registration form per child. Registration implies a beginning of the year start date. Families have the opportunity to add/drop through out the year (see enrollment agreement at the bottom of this form.)
Billing for Extended Day will be invoiced and due one month prior to services.
Registration will be open until August 31, 2018 or until the program is full.
1. If my child withdraws enrollment, the parent(s) or guardian financially responsible for him/her must submit a written cancellation notice to the office two weeks prior to withdrawal. Please note, for families that pay with the autopay option, the change will be applied retroactively with the following month’s deductions. To avoid this please notify Sunny Hollow of any add/drop more than 10 days before the date of the withdrawal.
2. Continued enrollment is contingent upon payment of extended day fees. If the extended day fee is delinquent and payment arrangements have not been approved by the Administration, the student will be dismissed from the program. Tuition for Before / After School Care is due by the due date on the invoice.
3. Prompt pick up is required. Please refer to the Parent Handbook for requesting drop-in care and late charge for pick-up policy.
