If you would like to officiate at the meet, please complete all sections of this form.  This meet will also be designated as an OQM, i.e., Official Qualifying Meet, for USAS National Certification as N3 level for all positions (pending evaluator availability). 

Application Deadline for:
  • Assigned Positions (DR, AR, SR, and CJ): 2/22/24
  • National Evaluation Applications: 2/29/24
