STEM Lending Library Reservation Request
This form is for schools to request devices from AoP Tech's STEM Lending Library. Please fill out this form completely and preview the calendar on our website for general device availability. Please note that the calendar and this request form should be used a general guideline and not confirmed availability.

Schools are permitted to borrow one STEM Lending Library device at a time for a single school week or 2 school week period. The coordinating educator must plan for pick up and device returning at the Brother's House on the campus of Archbishop Carroll. Coordinating educators will meet with a Technology Integration Coach for approximately one hour to review the usage and curriculum integration of the device. Additionally, the educator will be asked to sign a "Device Breakage Form" in the event that parts of the tools are missing or broken upon return.
