I understand that I am entering into a binding agreement for private dog training with K9 Parent Training LLC. I hereby waive and release K9 Parent Training LLC from any and all liability of any kind. This includes any damage or injury that could be sustained during the training process to the dog, person training, or anyone associated with the person who is training. This includes during the actual training session with K9 Parent Training LLC, both in the client home and our training facility, as well as outside of these training sessions.

I assume all risk for my actions and the actions of my dog. K9 Parent Training LLC does not guarantee the behavior of the dog at any time before, during, or after training and is not responsible for any actions of the dog before, during, or after training.

I am responsible for my dog’s health and well-being and overall control before, during, and after any training sessions. I assume all risk of any equipment malfunction such as broken collars, dropped leashes, or a dog that escapes the equipment. I understand my dog may have exposure to other dogs during the training process.

I understand completely that K9 Parent Training LLC does not guarantee, implied or otherwise, the success of training for my dog. All dogs are individuals and the efficacy of the training will be my responsibility as my dog’s owner and advocate. I am directly in charge of following through with training to see results.

I certify that my dog is healthy, has no contagious or communicable diseases, and is current on age appropriate vaccinations. If my dog becomes sick or contagious, I will notify K9 Parent Training immediately and reschedule any training sessions.

It is my responsibility to follow through with scheduled training sessions. If an emergency occurs, allowance may be made to change days and times within a reasonable time period (24-hour notice). If an emergency occurs within a few hours of our appointment, I will be given the chance to reschedule (determination of emergency is defined at the trainer’s discretion). If I have scheduled an appointment with K9 Parent Training, and I am not available or at the defined meeting spot, this session will be forfeited and treated as a training session. Payment for training session packages and programs are required in order to complete scheduling on our calendar.

I take full responsibility for compliance with all provisions of this agreement. 

I hereby authorize K9 Parent Training LLC to photograph me, take motion pictures of me, take video footage of me, and/or make electronic sound recordings of me (herein referred to as photographic or electronic reproductions).

I authorize the use of any such photographic or electronic reproductions of me for any purpose, including, but not limited to educational and other public media as may be deemed appropriate by K9 Parent Training LLC. I understand that I may be identifiable from such photographic or electronic reproduction. I understand that this includes my pet(s).
