2017-2018 Student Organization Registration Form
All current TCU recognized (funded or non-funded) student organizations and Graduate Student Organizations must register with the Office for Campus Life at the beginning of each academic year. Please have only one member of your organization complete the following registration.
NOTE: It is also required of other student organizations register as well, including Sub-Groups, Greek Organizations,and student organizations operating through various Campus Departments. If you have any questions please contact Joe Golia in the Office for Campus Life at ocl@tufts.edu.
NOTE: All student organizations are responsible for the guidelines, policies and regulations as stated in the following publications; The Campus Life website, the Student Handook through the Office of Student Affairs, the Tufts Community Union (TCU) constitution and bylaws and the TCUJ (Judiciary) constitution and bylaws. In completing this annual registration form your organization has agreed to these guidelines.

SPACE and RESOURCE RESERVATION SYSTEM CONTACT INFORMATION
Each student organization should have one designated contact for scheduling. This can be in addition to the primary and secondary contact listed above. Scheduling correspondence will be through Tufts email. Designated contacts can access the scheduling system using their Tufts username and password. Once this form is completed information will be sent to the scheduling office. Within 10 days of completing this form the scheduling contact, as well as the primary and secondary contact persons will receive scheduling access.

ANTI-HAZING AGREEMENT
As required by law, Tufts University must issue a copy of MGL c. 269, ss. 17,18, & 19 to all recognized, registered student organizations, both funded and non-funded. Furthermore, your acknowledgement of these regulations also includes the responsibility for notifying all members and potential members of your organization of these statutes. You may do this by reading the statutes at a general meeting of your organization. Tufts has a broader definition of hazing than Massachusetts law. Hazing is defined as any action taken or situation created, intentionally, whether on or off campus, to produce mental or physical discomfort, embarrassment, harassment, or ridicule. Such activities or situations may include but are not limited to the following: use of alcohol and/or controlled substances; forced consumption of any substance; paddling in any form; creation of excessive fatigue; physical and psychological shocks; quests; treasure hunts; scavenger hunts; road trips; or any other such activities carried on outside or inside the confines of the chapter house; wearing publicly apparel that is conspicuous and not normally in good taste; engaging in public stunts and buffoonery; morally degrading or humiliating games and activities; late work sessions that interfere with scholastic activities; and any other activities that are not consistent with academic achievement, fraternal law, ritual, or policy, or the regulations and policies of the educational institution or applicable state law.

Hazing also includes pressuring students to drink alcohol or other substances by means of drinking games or contests, or creating an atmosphere in which students feel that drinking is encouraged or expected, and that declining a drink would subject them to ridicule or criticism. Note that consent of participants is not an available defense against any prosecution for hazing.

University regulations prohibit hazing, as do the policies of every international fraternity and sorority adopted by the Inter-Greek Council at Tufts. Every member of any student organization, athletic team, and fraternity/sorority is individually accountable under the higher standards of the Tufts Hazing Policy. Responsible parties may include members, new members, officers, and the organization itself. Under the law, Tufts must, and does, notify students of the law, assign disciplinary consequences for a violation and report regularly to the Massachusetts Board of Regents of Higher Education.

I have read the MA and Tufts Hazing policies. I therefore agree to abide by it and assume responsibility for notifying all members of the above named organization. In addition, I understand that the act of registering implies that a student organization obligates itself to abide by all the rules and regulations of Tufts University pertaining to student organizations outlined on the Student Affairs and Office for Campus Life websites. Also, please note that registration does not necessarily imply or indicate Tufts University sponsorship or approval of the activities of the organization.
