By checking this box, I hereby give my consent for my child, named below, to participate in the Bite2Go program sponsored by Rural Resources, Second Harvest, and At The Core. I understand my child will receive weekly food packages as resources permit. If at any time I no longer wish my child to participate in the program, I will notify my child's school in writing. I understand that it is my responsibility to make sure that my child, named below, as well as any and all other residing with my child can safely eat the food distributed. I understand that I should consult a medical professional if I have any concerns regarding food allergies and my child/family's ability to consume any of the food provided. *
