Mt. Vernon Intermediate School Car Rider Tag Registration Form
Please fill out this form to register your student(s) so that they may be picked up at MVIS. Please fill out your MVIS student's name and grade level. If you have multiple students at MVIS, you can fill out their names/grade levels, too. 

After filling out the form, you can pick up your car rider tag from:
- MVIS Office
- MVIS Open House Night (Monday, August 4th) from the transportation table

This tag will help coordinate the organization and dismissal of students in the car rider line. Please place the tag on your mirror during PM pickup. The tag will not be required for AM drop off.
