Please use this form ONLY if the student rides in a SMALL BUS, VAN, SUV or PPS SEDAN.
If the student rides in a PRIVATE CAB (Radio Cab, Yellow Cab, Broadway Cab etc.), please contact Student Transportation at the number listed below.
Please provide at least 4 hours notice if possible.
If it is unknown how long the student will not require transportation, please submit a one day PAUSE request, and fill out a new form each day as needed.
