Waiver and Parent Signature *
I hereby submit my application for registration in the American Masters Martial Arts Tournament. I do hereby, for myself, my heirs, executors and administrators, waive, release and agree to indemnify and save harmless all persons connected with this American Masters Martial Arts Tournament, to include the all American Masters Martial Arts Academies, and any and all of their designated agents, forever discharge any and all rights and claims for damages from any and all losses caused by accident or injury that I may sustain, regardless of fault. Likewise, I take full responsibility for all my actions in connection with this tournament, and will follow the tournament rules and regulations, including wearing the required equipment, and understand I am fully bound by the legal definition of "Assumption of Risk", as I know this is a contact sport. At any time I have the right to stop my participation in any of the events and may voice my opinion on safety matters to a Head Judge or the Tournament Director. I understand that any pictures, video, or any other form of recording taken of me while participating in said tournament may be used for publicity without compensation. I also understand the entry fee is non-refundable unless prior arrangements have been made in writing, and are not contingent on whether or not I attend or compete in said tournament.
