Consent to display your modern name on Drachenwald's OP
The Order of Precedence (OP), which is maintained by the Posthorn herald, lists awards received by individuals living in the kingdom of Drachenwald. Awards are always listed under the person's SCA name.

The OP is a useful tool to recommend people for awards. All modern names will be hidden unless the person has given their consent for it to be searchable.

Please let Posthorn know whether or not your modern name may be published in the OP by filling out this form. This form contains three pages:

1) Information and consent regarding your modern name.
2) Options for providing your persona's preferred pronouns and title(s).
3) The confirmation page for filling out the form.

If you give your consent, then your modern name can either be displayed on a separate page from your SCA name, or on the same page together with your SCA name.

The following information is only accessible by relevant Drachenwald officers, that is the Posthorn herald and Kingdom webminister. In future, they may appoint deputies to assist with managing the OP database or to collect input to this form. The information will be stored in the OP database as long as award entries exist for that individual.
