I give permission to the Northwood Public School to release directory information, as defined in Northwood Public School’s record policy. This would include the school yearbook and newspaper. Things such as good deeds and students of the month, class activities, and other information about students may be included in the media. *
During the school year, we may take the students on an educational field trip during the school day. I give my permission for my son/daughter to attend this field trip. I have the right to revoke this permission at a later date.

If your child will be picked up, please list anyone you DO NOT want to pick up your child. 
Rural bus riders K-3: For your child's safety and welfare, NPS drivers must make visual contact with a parent/guardian, babysitter, or older sibling bus rider. Please acknowledge the bus driver.

If there is no one home to recieve the student, the bus driver will make an attempt to contact you. If unable to reach you, the student will ride the rest of the route. At the end of the route, the driver will make another attempt to contact you. If the driver still cannot reach you, the student will be brought back to the school and stay with personnel until you can be reached. 
Rural Bus Riders 4-6: I authorize NPS to leave my child home alone, without a parent/guardian or caretaker present and will assume total responsibility for the student. I will not hold NPS or the driver responsible if no one is home. If I do not feel that my child can be left alone, I will communicate that to the bus driver. 
I have filled out transportition information to the best of my knowledge at this time. Should any changes to after school transportation need to be made, it is my responsibility to notify the front office or classroom teacher of these changes.  *
