What do you plan to sell at the market? Please list all items that you think you will sell.
How long have you been producing your product(s)?
Do you have product liability insurance (required by the City of Burlington)?
If you're serving prepared foods, have you been inspected by the health department? Please include date.
Can you describe your set-up? Please include dimensions where possible.
For example, do you have a cart? One tent, two tents etc.
How often would you like to attend the market?
Extended Season (June 6th - October 31st)
Standard Season (July 11th - September 26th)
Half Season (10 times of your choice)
Day Vendor (Must schedule with Market Manager)
Please include a link to your website or Facebook if you have one.
Is there anything else that you want us to know when considering your application?
