Please complete this form if your agency would like to participate in this year's Community Service Week (October 21-28). Please fill out one form per project.

Projects may apply for limited reimbursement for project materials. Projects applying for funding must be recruiting volunteers, located in Montgomery County, serving Montgomery County residents. (Items not eligible for funding include food and marketing materials.)
