Please complete the following form to begin your equipment lending request. Orders should be placed no less than one week before the requested date of delivery if possible. Equipment deliveries and pick-ups to pK-12 schools and programs take place on TUESDAYS only each week. Equipment is typically loaned in two-week increments. Home-school families are welcome to borrow items for a one week time period. We are unable to offer delivery of our items to home-school/virtual families, but items can be picked up from our offices on the third floor of the Health Professions building on TUESDAYS ONLY.
