MOBILE DIAPER PANTRY PROGRAM – CLIENT INTAKE FORM & DISTRIBUTION POLICY
Parents who are requesting diapers must bring the required identification for themselves (driver's license, state ID, military ID, passport, VISA) and each child (certified birth certificate). Once all documentation has been verified and the family has been entered into our system, it will not be requested again unless re-verification is necessary.

Why do we ask families to register?
Registration helps us serve you better, make sure we have enough diapers for your children, and gather the information necessary to expand this program to more families. We are also required to report on our diaper distribution to grant funders and the National Diaper Bank Network (NDBN). Twice As Nice Mother & Child will keep your information private. We will not share your name, address, phone number, email address, or personal information. Data reported to funding agencies and the NDBN will only include group demographics (i.e. % of military families, etc). Each family who registers and visits the Twice As Nice Mother & Child Mobile Diaper Pantry will receive free disposable diapers and wipes for their children. Children in diaper sizes newborn through 4 will receive 50 diapers. Children in diaper sizes 5 and 6 will receive 25 diapers. Children in Pull-Ups will receive 25 Pull-Ups. Quantities of diapers and pull-ups provided are subject to change at any time without notice. Wipes may not always be available. We do not provide diapers for expecting families.

We only distribute diapers on the designated mobile pantry days. The schedule with dates, times and locations is available on our website under Mobile Diaper Pantry. Schedules are also available at each distribution site.
The Twice As Nice Mother & Child Mobile Diaper Pantry was created to provide free disposable diapers to families struggling to afford diapers for their children.

- I understand that the goal of the mobile diaper pantry is to serve families having difficulty affording diapers on their own.

Each household will receive one (1) supply of diapers and wipes for each child wearing diapers. We cannot provide a supply of diapers to multiple parents, caregivers, or grandparents for the same child. We do not provide diapers to expecting families.

- I understand that I cannot register multiple adults to receive diapers for the same child or request diapers if I am expecting a child.

When you visit the mobile diaper pantry, please bring identification for yourself and each child. Ex: driver's license, state ID, military ID, passport, VISA, certified birth certificate.

- I understand that I must bring ID for myself and each child to receive diapers.

To serve as many children as possible each month, we cannot provide diapers to unborn children.

- I understand that if I am pregnant, I cannot receive diapers until my child has been born.
