Rampart Locker Request, 2019-2020
A student must use this Locker Request Form if a student would like to be assigned a locker for the 2019-2020 school year. If you had a locker at Rampart in a previous school year, it is no longer your locker.

**If you do not want a locker you do not need to fill out this form!**

For new and returning Rampart students who want to have an assigned locker, please carefully read and fill out the form below to request a locker assignment. The form also allows you to request a locker partner. This form must by completed by 1 August 2019; if you miss this deadline, you can still request a locker using paper forms during Check-In Day on 6 August 2019, or in room 218 starting 9 August 2019.

Requested locker partners should be the same gender and grade level, unless you are siblings. BOTH students who want to be locker partners must submit an individual locker partner request in order to be assigned together. If only one student makes a request, that requested partner will not be assigned, but a locker will still be assigned to the requesting student. If the student ID number on the form does not match the student ID number in IC, your request will not be accepted.

You are allowed to submit ONE request. Once assigned, locker partner changes will ONLY be made at the discretion of RHS administration.

If, due to limited resources, Rampart needs to assign two students to a single locker, RHS staff will make every effort to assign students of the same grade and gender to a locker.

If you do not want a locker for the 2019-2020 school year, you DO NOT NEED to fill out this form. You will not have a locker assigned to you. However, if you change your mind during the school year, you can go to room 218 to have one assigned.

RHS administration will email your locker assignment and combination prior to Check-In Day on 6 August 2019; lockers WILL NOT be assigned during Check-In Day. If you do not receive an email, you can confirm your locker assignment at Check-In Day either at the Locker Confirmation table or by asking the library staff to print your class schedule, which will include your locker information.
