ELC Bullying Reporting Form

Bullying is an "IMBALANCE OF POWER when AGGRESSIVE BEHAVIOR is REPEATEDLY used to PHYSICALLY or EMOTIONALLY harm someone."


Under the Illinois School Code and Board of Education Policy 7:180, "Bullying" means any severe or pervasive physical or verbal act or conduct, including written or electronic communications directed towards a student that has or can be reasonably predicted to have the effect of one or more of the following:

  1. Placing the student in reasonable fear of harm to his/her person or property

  2. Causing a detrimental effect on the student's physical or mental health

  3. Substantially interfering with the student's academic performance

  4. Substantially interfering with the student's ability to participate in or benefit from the services activities or privileges provided by a school.

"Bullying" includes harassment, intimidation, retaliation, and school violence.

The ELC  School  Bullying Reporting Form is a digital reporting system for any type of bullying that has been experienced. The more information that you are able to provide, the more thoroughly we are able to investigate.


If you have observed or witnessed teasing, or know of a conflict that could escalate, please do not complete this form, instead, inform a teacher as soon as possible so he/she can intervene.

This form is for reporting acts of bullying as defined above so we can get help to those who need it most.

  • You can also call your local police department for assistance if your child or someone in your family is feeling unsafe or if you are worried about the safety of another. The police can help link people to mental health services and do a wellness check if appropriate. The non-emergency number for the Antioch Police is 847-395-8585.
