Halls High Chapter of the National Honor Society Request for Peer Tutor

Please read the following information before filling out the form.

GUIDELINES:

1. Specific peer tutors cannot be requested.

2. All tutoring must be completed on the grounds of the Hall High School campus.

3. All tutees must be current Halls High students.

4. Respectful, professional behavior is expected from the tutee and the tutor.

5. Failure of the tutee to contact the tutor after being contacted at least three times will result in the tutee being dropped from the program. Their school counselor will be contacted after the second time.

6. Failure of the tutee to notify the tutor of a cancellation or a "no show" three times will result in the tutee being dropped from the program. Their school counselor will be contacted after the second time.

7. All contact needs to be between the tutor and tutee. We request that parents not email nor text our student members. Our tutors will only communicate with their fellow students not adults.

INSTRUCTIONS:

1. Please fill out the following form to be matched with an NHS peer tutor. We only tutor academic subjects.

2. Please allow for three to five days for the match with a tutor and for contact to happen. Times will be longer over breaks. If you have not heard from your tutor to set up tutoring after five days from submitting this form, please contact your school counselor.

3. Check your school email for messages from your tutor while at school. Check your phone for texts from your tutor after school.They are required to contact you personally and will use both ways to contact you.
