UIL Computer Science: Parental Consent for Participation Form
As required by Texas Education Code §33.0815, parental consent is required for a student to participate in any district-authorized or district-sponsored club. A student is unable to participate in the student club until parental consent is provided. A separate form must be completed for each student club a student wishes to join. The club sponsor will keep the forms on file.
